Webinar Description

Supporting health, wellness, and aging in place for at-risk populations requires practical, person-centered, and interprofessional approaches. This webinar explores how digital health literacy and community asset mapping can be used to enhance access to care, promote self-management, and improve quality of life for community-dwelling adults.

Participants will examine how technology and community-based resources (both in-person and digital) can be leveraged to strengthen social connection, inclusion, and overall well-being. Emphasis is placed on understanding what matters most to individuals, assessing engagement in self-health management, and integrating wellness-focused strategies across the care continuum. The session highlights the role of interprofessional collaboration and transition planning in supporting sustainable, asset-based health promotion.

Presenter

Aimee Perron

PT, DPT

Aimee E. Perron, PT, DPT, has practiced for over 25 years as a clinician, educator, and leader in physical therapy across post-acute care and academic settings, with primary expertise in neurologic and geriatric physical therapy. She currently serves as Assistant Clinical Professor and Director of Clinical Education at Northeastern University and practices in post-acute care and pro bono clinics. Dr. Perron is committed to clinical excellence through evidence-based, person-centered care and is passionate about mentoring students and colleagues to advocate to ensure individuals have access to care and achieve meaningful, functional outcomes. She is a APTA board-certified Neurologic Clinical Specialist and a Certified Exercise Expert for the Aging Adult. She has presented nationally and internationally, including at multiple Combined Sections Meetings. Her scholarly interests include population health, and alternative models of care, including use of AI and telehealth. Dr. Perron serves on the APTA ANPT Practice Committee and is Vice President of APTA of Massachusetts and a Delegate to the APTA House of Delegate.
